It is very important for us to know your dental and medical history before any procedures can be performed. We also would appreciate knowing your dental insurance carrier (if you have one) in advance to help you understand your benefits for various procedures you may need. For your convenience, you can download the forms to fill out in advance then mail, fax or bring them with you. (All information is strictly confidential.)
If you are interested in becoming a patient and have no urgent needs, we would be glad to set up a hygiene appointment, and the doctor will review any concerns or conditions with you at that visit. If there is more extensive treatment needed, further appointments will be set up with the doctor to review and/or perform the needed work.
If you need to cancel your appointment with us, we kindly ask for 48 hour notice, so that we can allow that time for someone else.
